Normalize changelog (#265)
* Fix RST markup
This was formatted as markdown while this is a .rst
* Mention Issue/PR numbers in ChangeLog
* Rephrase changelog entries to be more succint
And clearer, IMHO.
* Move changes to Changed section.
* Remove redundant entry
